It includes the inflammatory reactions in the skin. They lead to the leakage of capillaries in dermis and result in an edema which continues till there is an interstitial fluid absorbed in the surrounding cells. It is also caused by the release of histamine and other inflammatory mediators like the cytokines from the cells in our skin. It can be non allergic or allergic and depends on the mechanism of release of histamine.

In the allergic disorder there is a release of histamine from the mast cells in skin. They are in response to the binding of allergen bound antigen. There are basophils and other cells which release the histamine and play a crucial role in the chronic utricaria disorder. In the auto immune disorder there are cases of chronic idiopathic utricaria which occur due to the auto immune trigger. 33 percent of the patients of chronic utricaria develop auto anti bodies which are directed at receptor FceR1 present on the skin mast cells./ The chronic stimulation of these receptors lead to the chronic hives. The patients also have other auto immune conditions like auto immune thyroiditis. In the infections there are hive like rashes which occur with the viral illness like common cold. They appear 3 to 5 days after the cold begins and appear till few days after the cold has gone.

In the non allergic urticaria the mechanisms other than allergen antibody interactions cause the release of histamine. It occurs from the mast cells. There are many drugs like morphine which leads to the direct release of histamine and do not involve the immune globin molecule. There are other groups of signaling substances which are known as the neuro peptides. They play a crucial role in the occurrence of emotion induced utricaria. There are dominantly inherited cutaneous and neuro cutaneous porphyries which are associated with the solar utricaria. They can be drug induced andare associated with the porphyrias and are caused due to the IgG binding and not IgE. There is a dietary histamine poisoning in which there is an ingestion of free histamine which is released due to the decay of bacteria. It is referred as the SCOMBROID FOOD POISONING. It is rapid in onset and has allergic type symptom complex which includes this disorder. But this type of utricaria does not include the wheels which denotes to the raised areas which are surrounded by the red base.

The cases of chronic idiopathic urticaria are related to the stress from the year 1940. There is large evidence which supports the association between this condition and emotional well being which denotes to the quality of life. There are few other studies which have stated that the stress as a causal factor in case of this disorder. There are evidence which have shown a link between the stressful life events and chronic idiopathic utricaria. There has been a little knowledge about the individual experiences and characters of the people which develop chronic idiopathic utricaria due to the stress. There has been an online project undertaken by the university of Plymouth.
