In older times it was known as the Bekhetervs disease. It involves the spine as well as pelvis. In few cases sacroilium is also involved. It may lead to fusion of the spine. It belongs to the family of Spondylo Arthropathies. It is linked to the genetics. If there is a complete fusion it can lead to the rigid spine and is known as the bamboo spine.

What are the signs and symptoms of Ankylosing spondylitis?

It includes the young males between the ages 18 to 30. At this age the symptoms are seen first. The other symptoms are chronic pain as well as stiffness in the lower area of spine. It can also involve the full spine in few cases. Sometimes the pain is referred to the back of thigh as well as buttock from the joint which is sacroiliac. It mainly involves the men and is quite painful in case of men as compared to the women. In few cases it may lead to inflammation of the eye and can involve the iris or uvea muscles of eye. Sometimes pain as well as swelling is seen in the case of ankles as well as feet. One can also see certain spurs. At rest pain can be severe and it improves as the person does more of physical work. The pain and inflammation is not constant and it keeps on changing. This condition is a combination of many disorders and is known as the sero negative spondylo arthropathies in which there are an inflammation of the pathological lesion.

How Ankylosing spondylitis can be diagnosed?

It includes the use of visual examination and a patient history. But there is no diagnostic test available which can be done, one must rely on the x ray and clinical examination. One must look for the spinal changes. The x ray diagnosis is such that the signs as well as symptoms of this condition cannot be known. It can be known after 8 to 10 years. There are few options for the diagnosis which includes the magnetic resonance imaging and tomography. These tests are not reliable. There is another test which is used to measure the flexion of spin and is known as the Schober test. In the acute phase of the condition the patient shows increase in the concentration of c reactive protein. There is also increase in the erythrocyte sedimentation rate which is known as the ESR.

In some cases the erythrocyte sedimentation rate and c reactive protein is same. It tells about the inflammatory state. But the inflammation rate can also be normal in the case even if there are high levels of the erythrocyte sedimentation rate and c reactive protein. One can also see the changes in gene but it is not a diagnostic test. The variation in the HLA B 27 can lead to increase in the risk in general population. It is seen in the case of blood tests.

How Ankylosing spondylitis can be treated?

There are few medicines available in the market and have given excellent results for it. One can also go for the exercise as well as physical therapy. Medical treatment must be done along with a physical as well as physio therapy. It decreases the inflammation as well as pain. The movements decrease the pain as well as inflammation. The exercise in the state of inflammation may lead to increase in the pain. The symptoms of the disease may also occur during normal activity. One may need the help of a cane so that the patient can walk properly. It relieves a pressure and helps to maintain the balance on affected joints. One must maintain a good posture which can decrease the chances of this disorder. The medicines which are required for this disorder are of following types such as the anti inflammatory drugs and opioid analgesics. They decrease the inflammation as well as pain and it includes the ibuprofen, indomethacin as well as the naproxen. The opoid analgesics have proved to be quite effective. They have proved to be quite effective in the case of chronic pain. The other medicines are TNF alpha blockers which act as very good immune suppressants and there are DMARDs which decrease the response to immune system. It occurs by the process of immune suppression.

The most effective treatment is seen in the form of TNF alpha blockers. They decrease the progress of disease by reduction in the pain as well as inflammation. They are quite effective in the case of spinal arthritis and they are quite expensive. The chances of infection are also increased with it. One must go for a test for tuberculosis before starting the treatment. It is known as the Monteux test. If the infection does not stop one must not go for the therapy and in some cases there is an immune suppression. The patients with TNF medicines must prevent their exposure to viral as well as fungal infections.

In severe cases one can go for the surgery. It is done in the case of joint replacements which include mainly the knee as well as hip. One can also go for the surgical correction of the spine as well as knee. One can also involve the neck but this procedure is quite risky. This disease can make anesthesia quite difficult. There can also be a problem in the intubation of epidural as well as spinal anesthesia. The problem may arise due to the calcification of ligaments. In few cases aortic regurgitation can also be seen. The ribs become hard and affect the lung function.

Rheumatologist must approve the physical therapies as the normal movements can also harm the patient. One must go for the physical manipulations as well as massage therapies.
